By submitting this form, you agree wit the policies of the Sac City Public Library and the laws of the State of Iowa.  I will promptly pay all fines charged to me for the loss or late return of materials.  I will give immediate notice of any change of address or phone information.  I will be responsible for all materials checked out to my children.  I understand that my child's selection of library materials will not be monitored by the library staff.  I understand that failure to return library matierials is considerd theft under Iowa law.
